If you are playing a game, and a new SEV patch comes out, can you patch it and keep playing, or do you need to start all over?

In most cases, you won't experience any problems continuing a previously saved game with a new patch. Usually it's known before the patch is released if its going to be saved-game compatible.
With respect to new mod versions, check with the mod's author(s) to see if it's saved game compatible.
